Do not stop after the employment application. Your application and authorizations are separate submissions. Use your full legal name and the same personal email address on each form so Human Resources can match them.
Your three steps
- Submit the employment application.
- Read and sign the separate background release.
- If you are applying for a direct-care role, read and sign the separate Colorado CAPS authorization.

If your role involves direct care to at-risk adults, you still need to submit the CAPS authorization. This includes caregiver and clinical positions. The CAPS form has its own signature and Submit button.
Applying for an office or administrative role with no direct care to at-risk adults? You may stop after the background release unless Human Resources asks you to complete CAPS.
Missed a step?
If you have not submitted the background release, complete the background release first. If you have not submitted the application, start with the employment application. Each form must be submitted separately.
