Home Health Aide (CNA)
CNAs provide direct personal patient care in a patient’s home according to a patient’s specific plan of care prepared by the RN/Case Manager.
CNAs have the ability to choose their workload and collaborate to find a schedule that works around other commitments.
Nursing students are encouraged to apply as this position counts toward patient contact hours for most programs.

Occupational Therapist (OT)
This position travels to adult and pediatric patients homes and assists with hands on therapy to expedite healing and recovery.
This position has a great deal of flexibility and allow you to create your own schedule.
Prior pediatric experience and/or comfort working with pediatric patients preferred. Will train the right candidate.
Applicants should have availability in their schedule to see at least 3-5 patients / complete 10-15 visits per week.
Pay rates:
Visit rates: $68.60-$105/visit (based on type of visit and dependent upon experience)
Hourly rate for meetings, case conference, training, orientation: $34.30-$44.10
Responsibilities:
- Performs comprehensive initial and on-going occupational therapy assessments on each assigned agency patient and develops the plan of care in collaboration with patient, RN Case Manager, and physician.
- Regularly evaluates the effectiveness of each assigned patient’s plan of care, response to treatment, revision needs, discharge plans, and progress to patient goals.
- Establishes goals to enhance the rehabilitative ability of the patient, which may include but not be limited to: neuromuscular facilitation/inhibition; perceptual, prosthetic, and kinetic treatment; orthotics/splints; activity of daily living training; and related to needs.
- Evaluates the safety of the home environment and recommends to patient/representative potential solutions for identified concerns.

Physical Therapist (PRN)
The Physical Therapist will be partnering with the primary care physician to create a care plan that reflects the patient’s needs and physician’s goals for recovery. Take advantage of the ability to make your own schedule and make a true difference in the lives of those for which you’re caring.
Prior pediatric experience and/or comfort working with pediatric patients preferred. Will train the right candidate.
Oasis training available as needed, but the majority of visits will be routine visits and evaluations/recertifications.
Applicants should have multiple weekdays of regular daytime availability in their schedule to see at least 3-5 patients / complete 10-15 visits per week.
Pay rates:
Visit rates: $95-$130/visit (**Note this is not per hour and is based on type of visit and dependent upon experience)

Physical Therapist (full-time)
The Physical Therapist will be partnering with the primary care physician to create a care plan that reflects the patient’s needs and physician’s goals for recovery and make a true difference in the lives of those for which you’re caring.

Oasis training available as needed, but the majority of visits will be routine visits and evaluations/recertifications.
Responsibilities:
- Performs comprehensive initial and on-going physical therapy services for each assigned agency patient and develops the plan of care in collaboration with patient, RN/Case Manager, and physician.
- Regularly evaluates and documents the effectiveness of each assigned patient’s plan of care, patient responses, revision needs, discharge plans, and progress to patient goals.
- Based on patient needs, seeks physician orders for equipment to promote patient comfort and safety, and when ordered and received by patient, initiates equipment use training.
- Evaluates the safety of the home environment and recommends to patient/representative potential solutions for identified concerns.
- Establishes therapy goals to enhance the rehabilitative ability of the patient, which may include but not be limited to: hot packs, gait training, diathermy treatments, therapeutic exercises, passive and active range of motion exercises, whirlpool baths and other related treatments to meet the agency patient’s needs.
- Meets productivity requirements established for home health care physical therapist (28 points per week determined by type of visit/activity)..
Pay rate: $83,300 – $112,700/year (dependent on experience)

Registered Nurse
The RN provides direct care to adult and pediatric patients within the home setting to ensure safe recovery from illnesses and injuries. You will work closely with all members of our nursing and therapy team to ensure the best possible outcomes for patients. Patients’ needs include, but are not limited to:
- Wound Care
- Wound Vac
- Therapy IV
- Antibiotic therapy
- Medication management
- CNA supervision
Additionally this role is part of our on-call rotation.
Multiple days a week of availability is preferred.
Pay rates (dependent upon experience):
Visit rates: $53.90-$98/visit (based on type of visit and dependent upon experience)
Hourly rate for meetings, case conference, training, orientation: $29.40-$34.30

Office:
Human Resources Specialist (part-time)
The Human Resources Specialist is responsible for administering all human resource functions of the organization and payroll processing. This position ensures the efficient completion of day to day office tasks and facilitates a positive, supportive work environment for employees both in the office and in the field.
QUALIFICATIONS:
- Three to five years experience in human resource management, preferably in home health care or related industry.
- Associates degree in Human Resource Management or equivalent combination of education and experience. Bachelor’s degree in Human Resource Management or related field preferred.
- Prior payroll processing experience.
- PHR or SHRM-CP professional certification(s) preferred.
RESPONSIBILITIES:
- Administers all aspects of the human resource function which include recruitment, benefits administration, compensation, organization development, employee relations, performance management, workers’ compensation and benefits.
- Manages biweekly payroll process in compliance with state and federal regulations, including wages, salaries, tax withholding, garnishments and benefits, adding and maintaining employee data and acts as main point of contact with payroll provider.
- Manages the recruitment and selection process of all staff, including developing and/or updating job descriptions, composing and placing online advertisements, interviewing candidates, extending offers, conducting background & reference checks, Fair Credit Reporting Act notices (as applicable), and new employee orientation.
- Oversees benefits administration, which includes collaborating with the agency’s broker on plan design, employee communication, website updates, new hire & open enrollment, required notices, life event changes, leaves of absence (paid sick leave, FMLA, FAMLI, ADA), COBRA and 401(k) plan management & employee eligibility.
- Coaches and partners with managers on employee relations issues, performance management and the progressive discipline process. Conducts workplace investigations as needed and escalates legal or high-risk matters to the Administrator
Pay Rate: $30-$40 per hour (dependent on experience)
Note: position is Monday- Friday 20 hours/week with some flexibility, but we request a regular predictable schedule spread throughout the week.

Non-Medical:
Personal Care Worker
Personal Care Workers (PCWs) work with the elderly and people with disabilities in an independent living community to promote safe and healthy daily activities. This position is responsible for providing hands-on personal care to participants within the participant’s residence. Additionally PCWs may provide homemaking services and respite care in order to allow primary caregivers a break from the responsibilities of caregiving. Duties include, but are not limited to:
- companionship
- meal preparation
- getting groceries
- assisting with ADLs
- light housekeeping
- assisting with walking and transferring
- assisting with bathing
- assisting with personal care
- respite care
Note: a majority of the work will be within one community. We do not have overnight shifts.
A valid driver’s license and reliable vehicle are required for this position and candidates must be willing to drive participants in their own vehicle.
**Must be able to work a minimum of 20 hours per week
Pay rates: $17.00-$24.18/hour (depends on type of visit)
Note: not all visit types may be available.
